AI Frontdesk → Intake Forms.
- Build a form field by field, or have HealOS extract one from a photo of your existing paper form and then edit it.
- Group forms into Appointment Categories and set per-doctor links, so a new-patient orthopaedic visit and a follow-up get different forms.
- Submissions lists what patients sent back. Approving a submission creates or links the patient.
How a patient gets the link
With Send Intake Forms on, the AI texts the link to new patients after a call, and emails it as well if the caller gives an email address. Staff can also send one by hand from Intake Forms → Send Form with a phone number, an email address, or both.
Links expire after a set time. A patient who waited too long can ask for a new one, or you can resend it.
Forms going to the wrong people
If patients who cancelled an appointment receive an intake form, the automation's Trigger Condition is set to "All callers". Set it to New patient calls only.
